LECOM is once again offering free physicals to youth and adult leaders, especially those planning to attend a long-term (Boy Scout summer camp/Cub Scout resident camp) or high adventure camp.
The physicals will take place at the Lake Erie College of Medicine (LECOM), 5000 Lakewood Ranch Blvd. on Saturday, April 21st from 9:00 a.m. until 3:00 p.m.

There are two Google Sheets worksheets:
1) Sign up in the cells for the times you wish to have the physicals done. There will be eight examination rooms, so 8 people may be examined each time slot. It is VERY IMPORTANT that you be ON TIME for your appointment. Appointment cancellations should be made no later than 48 hours in advance. A SCOUT IS TRUSTWORTHY AND COURTEOUS. The doctors and medical students are volunteers and their time and commitment should be respected.
2) The second sheet (see bottom tabs) is for entering contact information: Scout Name, Parent Name, email address, and phone #.
